NHS regulations state that all patients are to have an Allocated GP responsible for their care. 

This will NOT affect which doctor you can see on a day to day basis. 

Practice Etiquette

PLEASE SWITCH OFF MOBILE PHONES WHILST ON THE PREMISES.
PLEASE LEAVE PRAMS AND PUSHCHAIRS IN THE COVERED AREA PROVIDED OUTSIDE THE SURGERY.
PLEASE DO NOT SMOKE ON THE PREMISES.

Patient Data

Under the Data Protection Act 1998, you are entitled to access your clinical records or any other personal information held about you.  Please ask reception for charges.

Violent and Abusive Patients

This practice operates a zero tolerance to violence.  Patients who are violent or abusive to the GP’s or members of their staff will be removed from our practice patient list.

Accident & Emergency

The nearest Casualty Departments are Darent Valley Hospital and Queen Elizabeth Hospital.  There are also Urgent Care Centres at Queen Mary’s Hospital, which is open 24 hours, 7 days a week and at Erith Hospital, which is open from 8am to 10pm, 7 days a week.

Access for the Disabled

There are wheelchair access, parking and toilet facilities for the disabled at the surgery.

Chaperones

If you would like a chaperone to be present during a consultation please inform the receptionist either at the time of booking the appointment or when you arrive for your appointment at the surgery.

Parking

Slade Green – We would ask that you do not park in the spaces marked “DISABLED” at the front of the premises.  We do not accept responsibility for any damage caused to cars parked in our car park.

Colyers Lane – There is a patient car park and a staff car park.  We kindly request that all patients park in the patient car park.  We do not accept responsibility for any damage caused to cars parked in our car park.
